New Patients
Practice Area and Registration
In order to offer the most effective services to our patients we have a
restricted practice area from which we will take new patients on. At
this time the practice can only accept new patients whose permanent
residence is an address of the Warwick Gates estate.
Prospective patients should come in person to the practice to collect a
New Patient Registration Form and a GMS100 form. This can be completed
either at the practice or at home. These need to be handed into the
practice to complete the registration.
An appointment with a nurse may be necessary if you are over 44 and
on any repeat medication.
Please note once you are registered with our practice if you change your
name, address or telephone number would you inform one of our
receptionists including your postcode. If you move outside the practice
boundary area (Warwick Gates Estate) you will need to change your
doctor.
